Cardiovascular Manifestations in Systemic Lupus Erythematosus: A Clinical Analysis of 1,125 Patients with SLE

Abstract

Systemic lupus erythematosus (SLE), which is an inflammatory disease of unknown cause, is a representative autoimmune disease, which results in the production of autoantibodies, activation of the complement system and immune complex generation. SLE has multisystem organ involvement with a predilection for females and with a predominant age of onset from twenties to forties. Multisystem organ involvement includes cardiovascular manifestations as well as renal, central nervous, pulmonary, cutaneous manifestations, etc. Cardiac manifestations include pericarditis, myocarditis, endocarditis, myocardial infarction and coronary artery disease, and vascular manifestations include pulmonary hypertension, cerebrovascular disease, intestinal haemorrhage/perforation due to vasculitis and peripheral vasculopathies such as livedo reticularis, subcutaneous nodule, skin ulcers, digital gangrene, thrombophrebitis and Raynaud’s phenomenon. In this article, the characteristics of cardiac and vascular manifestations mentioned above, and treatment and prognosis in these diseases based on the clinical data from 1,126 patients with SLE fulfilling four or more of the revised ACR (American College of Rheumatology) criteria, who were examined and treated at the Department of Internal Medicine and Rheumatology in Juntendo University Hospital between 1955 and 2002, will be presented.
