Cost Effectiveness of Bariatric Surgery as A Treatment for Morbid Obesity: A Literature Review

Abstract

Bariatric or weight loss surgery has gained worldwide acceptance as an effective treatment for morbid obesity. The increasing number of bariatric procedures being performed globally has significant cost implications on health economy. This has prompted a stringent economic evaluation of the short and long term cost effectiveness. This review of the latest literature presents a summary of published data available on the cost effectiveness of bariatric surgery. Analysis shows that bariatric surgery is cost effective compared to conventional treatment or no treatment, particularly in severely obese and diabetic patients with convincing incremental cost-utility ratios (ICUR) ranging from $1,000 – $40,000 per quality adjusted life years. The initial costs of surgery can be amortised over 3.5 years. Roux-en-Y gastric bypass alone reduces 29% of total direct health care costs and adjustable gastric band has led to increased quality adjusted life years gained and cost savings compared to non-surgical management in the diabetic population. No definitive conclusion can yet be drawn from the cost effectiveness of different bariatric procedure. Taking into account the indirect economic impact of bariatric surgery on workers’ productivity along with the direct impact on health services, the overall economic impact of bariatric surgery is considerably profound.
