Magnetic Resonance Imaging in Guidance and Assessement of Cardiovasular Interventions

Abstract

Cardiovascular diseases are a major public and economic health problem leading to more than 7 million deaths world wide each year. These diseases require noninvasive minimal invasive or intensive interventions. X-ray fluoroscopy is considered the current modality for monitoring the interventions. It provides excellent spatial and temporal resolution, but is limited by exposure of patients and staff to ionizing radiation, poor soft tissue characterization and lack of quantitative physiologic information. MR-guided cardiac and vascular interventions have gained popularity in recent years. Clinical and experimental studies performed under MR-guidance have indicated the suitability of this modality for: delivery of ASD closure, aortic valves, and endovascular stents (aortic, carotid, iliac, renal arteries, inferior vena cava). It aids in performing ablation, creation of hepatic shunts and local delivery of therapies. Steering catheters by remote control under MRI guidance offers unique challenges and opportunities. Development of MR compatible equipment and devices will widen the applications of vascular and cardiac interventions. At post-intervention, MRI provides wealth of physiologic and structural information. With the aids of MR contrast media, MRI has the potential for assessing vascular patency, perfusion and viability as well as the efficacy of newer therapies and devices. MR-guided interventions will establish the basis for minimally invasive surgeries that offer comfort to patients and reduce healthcare cost.
